Emergency Contact Information

Waiver & Photo Release

I hereby authorize the Volunteers at the 2025 LIM359 Youth Camp to act for me according to their best judgment in any emergency requiring medical attention, and herby waive and release the camp and staff of all liability for any illness or injury of the participant incurred while at camp.

I grant to LIM359 the right to take photographs of me and my family in connection with the LIM359 Activities Camp. I authorize LIM359, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that LIM359 may use such photographs of me with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content.

Electronic Signature

Our camp is kept low cost for participants because of fundraising and donations. This is not mandatory, but please feel free to make a donation if you'd like.
